EN Travels

The following is a copy of the contract that all travelers must sign prior to the 2015 tour.

 

 

 

Parental Consent to Travel Form / Waiver of Liability / Contract

I, _______________________________, do hereby give permission for my son/daughter, ____________________,  to sign up to travel to Paris, Barcelona and London during the dates of June 11- June 19, 2015. 
 
I thoroughly understand the details of the trip as outlined through the informational meeting, online site, and/or provided materials.

 

I understand and agree to follow the payment plan as outlined on the tour website http://www.explorica.com/Schmiesing-7909.  Any changes/modifications needed will be made through the tour company Explorica.

 

I understand that my son/daughter must obtain a US Passport, the cost of which is separate from the tour price, and that there may be costs in addition to the quoted price for individual purposes and purchases.

 

I understand that signing this form does not guarantee a spot on the tour for my son/daughter, but only the intent and ability to travel.  Those selected to participate are at the complete discretion of the program organizer based upon a number of factors, including, but not limited to: the chaperones’ perception of student’s responsibility/maturity level, ability to work well with the others in the group and other relevant factors.   

 

If more students request and are approved to participate than there are spots available, it is possible that the positions will be awarded based on a first-come, first-serve basis, which will be announced to the potential travelers before the sign up process.  

 

I understand that my son/daughter must remain in good school standing (acceptable grades and behavior) or they could potentially be denied the opportunity to travel, with no refund of any payments made.  If, at any point prior to the dates of travel the traveler engages in any activity or demonstrates behavior that could be perceived by the program organizar, Abby Schmiesing, to be problematic, the traveler´s participation in the tour could be revoked, with no refund of any money paid toward the trip.  Such behavior could include, but is not limited to: suspension/expulsion from school and/or dangerous or illegal activity in or outside of the school.This measure would be taken after careful review of the situation by the school administration and the travel coordinator, Abby Schmiesing. 

 

I understand that, during the trip, should my son/daughter intentionally behave in a way to endanger themselves or others, or disobey specific instructions for their safety and that of others, that their activities during the trip may be restricted, or, in extreme circumstances, they could be sent back home on the next available flight, at their own expense.  All travelers, including adults, agree to abide by the laws of the United States of America primarily, in addition to any laws of the destination country/city.  Any questions of whether an activity or behavior is acceptable is to be directed to the travel coordinator, Abby Schmiesing.

 

I recognize that there are inherent risks associated with the activities as outlined on the trip itinerary, including but not limited to travel risks.  I hereby elect to allow my son/daughter to participate in said activity with full knowledge that said activity may be hazardous to me and my property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, PROPERTY DAMAGE OR PERSONAL INJURY, INCLUDING DEATH, that may be sustained, or any loss or damage to property owned by me, as a result of being engaged in such an activity, WHETHER CAUSED BY THE NEGLIGENCE OF RELEASEES or otherwise.

 

I further hereby AGREE TO INDEMNIFY AND HOLD HARMLESS the RELEASEES from any loss, liability, damage or costs, including court costs and attorney fees, that they may incur due to my participation in said activity, WHETHER CAUSED BY NEGLIGENCE OF RELEASEES or otherwise.

IN SIGNING THIS RELEASE, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Waiver of Liability and Hold Harmless Agreement, understand it and sign voluntarily as my own free act and deed; no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made; I am at least (18) years of age and fully competent; and I execute this Release for full, adequate and complete consideration fully intending to be bound by same.
